Аннотация

В работе делается попытка продемонстрировать механизмы, на разных этапах эволюционного развития человечества контролирующие структуру заболеваемости. Система реакции на стресс складывалась у млекопитающих с учетом их активного поведения, что позволяло при стрессе либо сражаться, либо бежать от опасности. Именно поэтому ранний ответ на стресс направлен на обеспечение мышц кислородом и необходимым питанием. Только в том случае, когда в силу обстоятельств этот механизм не срабатывал, включалась вторая фаза стрессового ответа, направленного на сохранение мозга в сложных условиях. Образ жизни современного человека изменился, но механизм реагирования на стресс остался старым. В момент переживания опасности человек не бежит, но стоит или сидит, то есть не двигается. Избыток адреналина, который не утилизируется мышцами в этих обстоятельствах, сигнализирует мозгу о крайней опасности ситуации, что ведет к началу реализации второй фазы стрессорного ответа. Все виды современных «болезней адаптации» являются следствием постоянного включения этой второй фазы стресса, не рассчитанной на работу в простых условиях.

В процессе эволюции сформировался эффективный механизм защиты развивающегося мозга от негативного влияния собственных гормонов стресса. На протяжении первых лет жизни обнаружена гипореспонсивность на стресс, которая позволяет мозгу интенсивно развиваться. Однако это явление встречается только в случае адекватной заботы о ребенке в семье и формирования безопасной привязанности. При неблагоприятных условиях раннего развития ребенка (например, пребывание в детском доме) гипореспонсивность не развивается, что ведет к срыву функционирования системы, отвечающей за стресс, и замедлению развития самого мозга. Эти изменения не заметны в раннем возрасте, но позднее проявляются в пубертатный период в виде неадекватного поведения подростка.

Таким образом, формирование тех или иных заболеваний в настоящее время предопределяется несоответствием поведения современного человека эволюционно сложившимся механизмам. Новая дисциплина — психология здоровья — призвана определять условия, при которых человек готов менять свое поведение в сторону здоровья.

Исследование в области психологии здоровья должно удовлетворять двум целям: развитию теоретических моделей, описывающих процессы, лежащие в основе здорового поведения и поведения, ведущего к риску развития заболевания, а также созданию эффективных процедур для изменения поведения, которые могут применяться в клинике и в целях профилактики заболеваемости.

В настоящее время можно выделить три основных направления психологических исследований в области здоровья: психофизиологическое исследование прямого влияния поведения на болезнь [3; 9], внедрение поведенческих методов для контроля этого прямого влияния [19; 23] и поведенческие исследования, нацеленные на профилактику и контроль хронических заболеваний [31].

Именно последнее направление представляет наибольшую значимость и с практической, и с теоретической сторон: очевидно, что профилактика приведет к снижению заболеваемости, а эффективная профилактика будет свидетельствовать об адекватности теоретических представлений реальности. В практической плоскости, например, больные диабетом должны придерживаться особой диеты, делать специальные упражнения по снижению веса, а также строго соблюдать предписание врача, что может в существенной мере снизить выраженность симптомов заболевания. Подобная ситуация должна быть и у людей, имеющих высокое кровяное давление: они должны соблюдать диету, снижающую вероятность появления атеросклеротических бляшек, двигаться не менее двух часов в день и т.д. Все это требует волевого решения от больного в контроле своего поведения, и здесь методы психологии здоровья могут быть весьма полезными.

В начале 20 столетия произошла резкая смена структуры заболеваемости, поскольку до этого времени большинство людей умирало от инфекционных заболеваний. Например, в конце 19 века летом в России погибало от кишечных инфекций до 85% новорожденных [6]. Открытие пенициллина А. Флемингом в 1928 г., а затем в нашей стране полученном в 1942 г. З.В. Ермольевой и Т.И. Балезиной привело к резкому изменению картины заболеваемости. Появились болезни, которые ранее не были описаны и встречались существенно реже, чем сейчас, поскольку дети рождались слабыми и рано погибали от инфекций. Так, Г. Аспергер ввел термин «аутистичные психопаты» в 1938 г., а Лео Каннер сообщил о «раннем детском аутизме» в 1943 г. — тогда, когда с помощью антибиотиков ослабленных детей с этим заболеванием научились выхаживать.

Однако уход от инфекционных заболеваний, которые на протяжении веков контролировали численность людей на земле, не означает полностью выход из-под действия естественного отбора. Эволюционное происхождение человека продолжает активно проявляться в структуре заболеваемости, которая обусловлена поведением современного человека, неадекватным физиологическим механизмам, возникшим в иные временные эпохи.

Сейчас основные заболевания, поменявшиеся местами с инфекционными заболеваниями как первыми в десятке болезней, ведущих к смерти — хронические расстройства адаптации: кардиоваскулярные заболевания и рак [20]. Недавний всплеск ВИЧ и распространения СПИД, как инфекционного заболевания, не исключает возможности движения в этом направлении, поскольку лечение ВИЧ-инфекции длительно, то есть является хроническим, и требует длительного интенсивного самоуправления [35]. Лечение хронических заболеваний сейчас составляет более половины бюджета развитых стран, направленного на здоровье [26], и поскольку хронические заболевания чаще встречаются в старшем возрасте, бюджет растет с возрастом популяции. Поэтому хронические заболевания связаны с основными проблемами общества и системой здравоохранения.

Поскольку поведение людей, включая потребление пищи, физическую активность, курение, причинно связано как с уязвимостью, так и с условиями поддержания хронических заболеваний, вполне оправдано сфокусироваться на профилактике заболеваний и контроле волевых свойств [9].

Опосредованность здоровья современного человека поведением в значительной мере предопределяется его эволюционным происхождением. Основные заболевания, приводящие к смерти, обычно, называются «болезнями адаптации» [2; 4].

Встает вполне правомерный вопрос, каким образом адаптация может приводить к смерти? Объяснение этому как раз и кроется в эволюционном процессе. Образно говоря, мозг современного человека находится в теле кроманьонца — тело современного человека фактически не изменилось за последние 60 тыс. лет. При отсутствии изменения физиологических механизмов существенно изменился образ жизни, особенно в последнее столетие, что и вызвало всплеск ранее редко встречающихся заболеваний.

Стресс у млекопитающих реализуется двумя различными, но взаимосвязанными системами: симпато-адреналовой и гипоталамо-гипофизарно-надпочечниковой. Симпатоадреналовая система является компонентом симпатического отдела вегетативной нервной системы, реализующей адреналин из мозгового слоя надпочечников. Нарастание в крови адреналина облегчает быструю мобилизацию метаболических ресурсов и ответа типа «борьба — бегство» [11]. Гипоталамо-гипофизарно-надпочечниковая система в конечном итоге вызывает выброс кортизола. Кортизол — стероидный гормон. В отличие от адреналина, который не пересекает гематоэнцефалический барьер, в значительной степени мозг является основной мишенью именно кортизола [10]. Также в отличие от норадреналина, кортизол примерно в течение 25 минут имеет пиковый уровень, и многие из его влияний на тело и мозг связаны с изменением генной экспрессии. Как следствие, влияние кортизола проявляется позднее и длится дольше [14].

Регуляция обеих звеньев связана с влияниями на уровне гипоталамуса, где интегрируются вегетативная и эндокринная функции с поведением [29]. Более того, вход в гипоталамические ядра, который обеспечивает гипоталамо-гипофизарно-надпочечниковые и симпатоадреналовые ответы на психологические стрессы, включает и кортико-лимбические волокна [21].

Возвращаясь к кроманьонцу, тело которого имеет современный человек, стоит припомнить, что его система ответа в сложных обстоятельствах соответствовала другим условиям жизни. При встрече с опасностью происходила активация симпатической нервной системы, что вело к притоку крови к мышцам и выбросу инсулина. Последний позволяет всем клеткам организма использовать сахар, который в обычных условиях может потреблять только мозг (в нем нет запасов сахара, в отличие от других органов). Все это предопределяло два поведенческих ответа на опасность: бегство или борьба. Если этот этап протекает успешно, то на этом и прекращается реакция на стресс. Но если опасность чрезмерно велика и первое действие не помогает, включается следующий этап ответа, который несколько отставлен, поскольку требуется время для передачи информации по цепочке гормонов. Это отставление важно, поскольку последствия от включения этого ответа крайне тяжелы и для самого организма.

Задача этого второго этапа, связанного с активацией гипоталамо-гипофизарно-надпочечниковой системы и, в конечном итоге, с выбросом гормона кортизола, состоит в том, чтобы поддержать активность мозга за счет отключения многих базовых механизмов организма и снижения активности других. Организм оценивает ситуацию как крайне опасную и все силы направляет на сохранение органа управления — мозга. Поскольку кортизол имеет рецепторы ко всем клеткам в организме, он сначала отключает самые дорогостоящие системы обеспечения жизнедеятельности: иммунную, восстановительную и половую, поскольку в момент опасности ими можно пренебречь. Затем идет подавление выброса инсулина, а потому сахар достается только мозгу. Мозгу требуется кислород, а потому дыхание становится более интенсивным (мы знаем, как глубоко вздыхает человек, переживающий, например, горе).

Для кроманьонца это была эффективная система встречи с опасностью. Вторая часть стрессового ответа включалась крайне редко — только тогда, когда силы организма были на исходе. Проблема возникла тогда, когда стресс стал по преимуществу психологическим, и, получая информацию, современный человек в силу обстоятельств не бежит и не сражается, но остается стоять на месте или сидит. Мышцы не поглощают адреналин. Лишний адреналин с током крови попадает в гипоталамус и оповещает о своем избытке центральную систему регуляции стрессом. Система настроена таким образом, что избыток адреналина в крови оценивается как чрезмерная опасность, что и ведет к включению второго этапа борьбы со стрессом в условиях, когда это не требуется объективными обстоятельствами. Более того, поскольку адреналин в крови остается, это состояние становится хроническим и ведет ко всему спектру основных болезней адаптации. Итак, отсутствие движения при восприятии эмоционального события, с которым легко можно справиться, ведет к неверной оценке ситуации стрессовой системой, настроенной на другие условия существования, что, в свою очередь, ведет к хроническому расстройству. Это хроническое расстройство будет обусловлено конкретными генетическими особенностями организма, общими же для всех людей являются такие изменения.

Хроническое снижение активности иммунной системы влечет за собой следующие события. В день у человека происходит примерно 6 млн. делений клеток. Это преимущественно клетки кожи и слизистых оболочек желудочно-кишечного тракта, подвергающиеся наиболее интенсивному внешнему воздействию, и клетки крови, в том числе лимфоциты, активно встречающиеся с инфекцией, и эритроциты, которые, будучи безъядерными, могут жить не более 3 месяцев. Известно, что на 1 млн. клеточных делений совершается одна ошибка. Следовательно, около 6 потенциально раковых клеток возникают у каждого человека ежедневно (этот процесс более интенсивен в детстве и замедлен в пожилом возрасте). В норме большие иммунные клетки макрофаги движутся внутри ткани и съедают все, что не соответствует критерию нормальной клетки, в том числе и потенциально раковые. Пока иммунная система находится в норме, она охраняет организм. В ситуациях, которые организм ошибочно (в силу неадекватного поведения человека, с точки зрения кроманьонца) принимает за чрезмерные, хроническое снижение активности иммунной системы ведет к раковым заболеваниям.

Длительное подавление активности иммунной системы при снятии стресса ведет не к нормализации активности, но к чрезмерной активации. Если в этот момент во внешней среде будет пыльца или даже просто домашняя пыль, активированная иммунная система воспримет этот объект как агрессор, и будет атаковать. Такова природа аллергических заболеваний, обусловленных стрессом.

При хроническом стрессе длительное подавление выброса инсулина способствует гибели бета-клеток островков Лангерганса, синтезирующих инсулин, и снижению реакции тканей на инсулин. Это, в свою очередь, ведет к развитию диабета второго типа, или диабета, связанного со стрессом.

Усиление клеточного дыхания в митохондриях приводит к появлению гипероксида — кислорода, наделенного лишним электроном. Он участвует в окислительном фосфорилировании, результатом которого является энергетические молекулы — АТФ. Однако когда стресс входит в хроническую стадию, гипероксид оказывается в избытке, он покидает клетку и как свободный радикал может повреждать любые молекулы, в том числе и стенки сосудов. Это заканчивается формированием атеросклеротических бляшек и в дальнейшем — ишемической болезнью сердца и ее осложнениями (инсультом и инфарктом).

Кортизол также способствует выбросу избыточного количества соляной кислоты в желудочно-кишечном тракте, что заканчивается язвами различных отделов этой структуры. Таким образом, мы описали все заболевания, стоящие на первых местах в списке смертности. Очевидно, что они возникают как результат неправильного поведения при переживании неприятных событий. Их легко «избежать» в прямом смысле слова бегом и ежедневными движениями. Нормой считается 2 часа ходьбы быстрым шагом ежедневно или замена ее активными физическими упражнениями большей интенсивности, но с меньшим временем проведения.

Современные данные, однако, обнаруживают дополнительные проблемы, связанные с системой стресса. Эти данные получены американскими исследователями при обследовании детей, взятых в семьи из России и Румынии. Особенностью этих детей было то, что они жили не в семьях, но в детских домах. Исследователи пытались понять, почему дети, усыновленные до полутора лет, имеют меньше проблем в подростковом возрасте в новой семье по сравнению с детьми, которых приняли в семьи после этого срока. Дети, усыновленные после этого периода, обязательно обнаруживали негативное поведение в подростковом возрасте, что приводило либо к возврату детей в детские дома, либо к смене приемной семьи [5].

Тщательные исследования здоровья приемных детей при передаче их из детского дома в семьи обнаружили существенные изменения в развитии структур головного мозга [12; 15; 16; 36; 38], которые свидетельствуют о замедлении их созревания после пережитого стресса в возрасте до 1 года [23]. Исследователям удалось доказать, что эти ранние изменения в дальнейшем предопределяют срыв процесса адаптации в подростковом возрасте [39].

Нарушение волевых функций, функций управления поведением — наиболее очевидные проблемы у детей-сирот. Они наблюдаются и в спокойном состоянии [17], и в состоянии эмоционального возбуждения [33]. Показано, что чем дольше ребенок находился в детском доме, тем менее он готов сдерживать себя [28]. Более того, у дошкольников, которые слишком малый срок были в родной семье и в дальнейшем жили только в детском доме, существенных изменений функций управления не наблюдалось даже через год после помещения в приемную семью [8]. Возможно, это связано с негативным влиянием стресса на поздно созревающие префронтальные области коры, которые в ранний период развития крайне чувствительны к негативным воздействиям.

Ухудшение работы префронтальных областей коры было показано с помощью разных методик [7; 36; 40].

Нарушение функций тормозного контроля особенно заметно у дошкольников и существенно зависит от времени принятия в семью: чем раньше ребенок оказался в приемной семье, тем лучше его тормозные функции [24]. Именно тормозные функции обеспечивают дальнейшее эффективное социальное взаимодействие и волевое регулирование поведения.

Среди биологических причин слома функций управления головного мозга может быть состояние систем, отвечающих за стрессовую реакцию. В исследованиях, проведенных на крысах, показано, что в первые две недели жизни невозможно вызывать повышение уровня кортизола на стрессоры [32]. Объяснение подобной гипореспонсивности состоит в том, что с помощью этого механизма организм защищает развивающийся мозг от потенциально опасных влияний повышенной концентрации кортизола [13; 37].

Более того, нормальные вариации материнства у крыс влияют на формирующуюся стрессовую систему [27]. В сравнении с теми самками, которые вылизывают и вычищают крысят реже, самки, делающие это часто, имеют потомство, которое позднее более бесстрашно исследует пространство и лучше выдерживает стресс [25].

Эти события фиксируются на молекулярном уровне, поскольку материнское вылизывание регулирует процесс метилирования генов рецепторов кортизола в некоторых структурах мозга [18]. Метилирование — это основной механизм закрытия генов.

Тщательное вылизывание снижает метилирование рецепторов кортизола [41]. Это приводит к тому, что большее число активных рецепторов сохраняется к тому периоду, когда организм выйдет из гипореактивности к стрессу и вынужден будет отвечать на внешнее воздействие. Следовательно, такой организм будет более эффективным в более зрелом возрасте [27]. Эти эпигенетические эффекты потенциально необратимы [41].

Для младенцев показаны эти же феномены. После рождения снижается уровень кортизола в случае эффективной заботы о ребенке [22] и формирования безопасной привязанности [42]. При небезопасных отношениях стрессоры могут вызывать нарастающий выброс кортизола, что свидетельствует об активации гипоталамо-гипофизарно-надпочечниковой системы. Подобная гипореактивность на стресс у детей продолжается вплоть до подросткового возраста. Наряду с психосоциальными процессами, связанным с переходом ребенка в пубертатный период, биологические процессы, обусловленные выбросом половых гормонов, меняют нейробиологию стресса в сторону взрослого ответа [35].

Итак, взрослые могут предотвратить рост уровня кортизола у ребенка даже во время стрессовых событий. Ответственное родительство позволяет детям звать на помощь, выражая негативные эмоции без изменения состояния эндокринной системы регуляции стресса. Напротив, неадекватное родительство может быть источником опасности и вести к изменению системы реагирования на стресс у ребенка [1; 30].

Гипореспонсивность на стрессовые события в период интенсивного роста — эволюционный признак, присутствующий у многих млекопитающих и предопределяющий эффективное развитие мозга в детстве и успешную его работу в период зрелости. Во времена естественного отбора этот механизм способствовал сохранению особей с созревшими мозговыми структурами. Частичный выход человека из-под естественного отбора и отсутствие учета особенностей развития детей ведет к тому, что высокий уровень кортизола в первые месяцы жизни ломает систему стресса и замедляет развитие мозговых структур, что долгое время остается незаметным. Очевидным оно становится в пубертатный период, когда выброс большого количества половых гормонов обнаруживает несостоятельность системы стресса и недостаточность системы управления собственным поведением. Восстановить эти функции уже практически невозможно, но профилактические действия по обучению родителей раннему общению с ребенком могут быть эффективны.

Изменение реагирования стрессовой системы может оказать влияние на эффективность любой адаптации в дальнейшем, оно влияет и на физиологический рост ребёнка, и его когнитивные функции и, в конечном итоге, здоровье. Следовательно, поведение родителей в данном случае предопределяет здоровье ребенка через способность управлять своим поведением.

Эволюционные корни современной заболеваемости можно проследить не только в особенностях работы системы стресса, в несоответствии поведения современного человека своему телу кроманьонца, но и в системе питания.

Неравномерность появления пищи у наших предков привела к жесткому механизму, когда редко получаемый продукт сразу отправлялся в систему хранения и был неприкосновенным до той поры, когда остро вставала проблема выживания. Жировые клетки не умирают, в отличие от нервных. Люди предпочитают при свободном выборе более жирную пищу. Жировые запасы человека не тратятся, если человек питается неравномерно. Это означает, что перекармливание детей с детства ведет к ожирению, которое будет сопровождать человека всю жизнь.

Питание должно зависеть от возраста — более обильное и жирное в раннем возрасте и менее калорийное после 40—50 лет, поскольку потребление пищи должно зависеть от активности человека. В процессе питания человек должен убеждать свой организм, что пищи много, а потому есть нужно небольшими порциями, но достаточно часто — 4—5 раз в день. Редкое и обильное потребление пищи будет вести к ожирению, частое, но маленькими порциями — к адекватному расходованию съеденного.

Итак, рассматривая эволюцию человека, можно объяснить многие болезни поведением, неадекватным физиологическим механизмам. Однако изменение поведения требует подключения механизмов мотивации и воли. Сейчас болезни адаптации иногда называют болезнями расстройства управления желаниями: нужно двигаться, ограничивать себя в пище, управлять своими эмоциями, чтобы быть здоровым. И здесь заканчивается психофизиология стресса и начинается новая наука — психология здоровья, суть которой состоит в создании условий для изменения поведения человека в сторону управления своими желаниями.

Abstract

In the article the attempt was made to show the evolutionary mechanisms’ control of the human being diseases structure previously and today. The mechanism of stress reactions emerged in the mammal with a glance of their active behavior, which allowed the under stress to fight or to fright. This is why the first answer on the stressor is directed to the muscles provision of oxygen and nutrition. But in that case these mechanisms have not worked the second answer switched on. This second answer was directed to the brain maintenance under complex circumstances. The way of life of modern human being differs from that of Cro-Magnon, but the stress mechanism is the same. In the moment of danger modern human being does not fight but is standing or sitting that is does not move. The excess of adrenalin which is not used with muscles signalizes to the brain that situation is extremely dangerous and the realization of the second stress answer begins. All types of modern «adaptation diseases» are the consequences of the permanent switching on the second stress answer but it was emerged just for extreme conditions.

In the evolutionary process the effective mechanisms of developing brain safeguard from the negative influence of the own stress hormones was originated. During the child’s first year of life the hyporesponsivity to stress is emerged which allows the brain to develop intensively. But this hyporesponsivity emerges just in the case of adequate child’s care in a family and safe attachment forming. Under adverse conditions of a child’s early development (for example living in orphanage) hyporesponsivity does not emerge, and this is followed by the stress system breaking and brain development retardation. These changings are not observed in early ages, but later could be seen in adolescence in a nonadequate teenager’s behavior.

Thereby the diseases which are forming now are predetermined by the discrepancy of modern human being behavior of the evolutionary emerged mechanisms. New discipline — health psychology — is called to identify the conditions which help a human being to change his (her) behavior to the health direction.

The scientific research in health psychology has to satisfy two purposes: the development of modern theoretical models describing the processes lying at the basis of health behavior and behavior following the high risk of diseases beginnings, and creation of the effective methods of behavior changing, which could be used in clinic and for preventive health care.

Now three basic directions of psychological research in health psychology can be sorted out: psychophysiological investigation of direct behavior influence on a disease [3; 9], introduction of the behavior methods for the monitoring of this direct influence [19; 23] and behavioral research aiming at the prophylaxis and chronicle diseases [31].

The last direction is the most significant both from theoretical and practical sides: it is obvious that prophylaxis follows the diseases’ decrease and effective prophylaxis could be evidence of theoretical concepts’ adequacy to the reality. In practice for example, diabetes patients must keep a special diet, do special exercises for weight loss and keep a doctor’s prescription which could decrease a symptom’s disease intensity. Similar situation could be for patients with high level of blood pressure: they need to keep a special diet, decreasing a probability of atherosclerosis plaques appearance, moving more than 2 hours per day and cetera. These kinds of behavior need volitional actions of patients for behavior control and the methods of health psychology could be useful to support these patients.

At the beginning of the 20th century sharp change of the diseases’ structure occurred, because before this period of time most people died from infectious diseases. For example, at the end of the 19th century in summer in Russia 85% newborns died from enteric infections [6]. Discovery of penicillin by A. Fleming in 1928 yr., and then in 1942 yr. in our country derived by Z.V. Ermolieva and T.I. Balesina resulted in sharp changing of the diseases’ picture. New diseases appeared which had never been described earlier and met significantly less rarely than now, because of those children were too weak and died from infections early. Then G. Asperger introduced the term "autistic psychopath" in 1938 yr., and Leo Kanner reported about "early child autism" in 1943 yr. when it was possible to take care of the weak children using antibiotics.

But escape of the infection diseases which during lots of centuries controlled the population size on the earth does not mean the avoidance of the natural selection activity. Natural human being origin manifests itself actively in the structure of modern disease incidence, which is determined by modern human being behavior, inadequate to physiological mechanisms originated in other periods of mankind.

Now the basic diseases change their places with infectious diseases as the first one in ten diseases followed by death are chronicle disorder of adaptation: cardiovascular diseases and cancer [20]. Resent surge of HIV and spread of AIDS as infection disease agrees with this tendency, because the treatment of HIV-infection is a long period of time, which is a chronicle one and need long intensive self-management [34]. Treatment of the chronicle diseases now averages more than half of industrial democracies’ budgets of public health services [26] and its increase is following the increase of mean age of population because chronicle diseases occur fairly often at elderly age. That is why the chronicle diseases are connected with the main problems of the society and the system of public health services.

People behavior (eating, physical activity, smoking) is causal connected both the vulnerability and chronicle diseases supporting. That is why we need to pay great attention to diseases prophylaxes and volitional control [9].

The mediation of the modern human health with his (her) behavior is the result of the evolutional origin of species. The main diseases followed by death are usually known as "adaptation diseases" [2; 4].

The lawful question is raised how the process of adaptation could lead to death? The Explanation is found out in evolutionary process. To express this figuratively brain of modern human being is situated in Cro-Magnon body, because the body has not been changing during the last 60 thousand years. In the absence thereof any physiological mechanism’s changing way of life changed considerably especially during the last century, and the surge of the diseases which occurred too rarely formerly is the result of this discrepancy.

Stress of the mammals realizes two different but interconnected systems: sympathoadrenal system and hypothalamo-pituitary-adrenal axis. The sympathoadrenal system is a component of a sympathical part of an autonomic nervous system releasing an adrenalin from medulla of adrenal glands. The adrenaline increase facilitates fast mobilization of metabolic resources and response "fight — flight" [11]. The result of hypothalamo-pituitary-adrenal axis’s activity is the cortisol overshoot. Cortisol is a steroid hormone. As opposed to adrenalin which does not cross hematoencephalic barrier cortisol aims at the brain [10]. As opposed to norepinephrine cortisol has maximal level during 25 minutes and a lot of influences on the body and brain are determined by gene’s expression. As a result of these differences the cortisol influence appears later and lasts longer [14].

Regulation of two parts of an autonomic system is connected with influences on a hypothalamic level where a vegetative and an endocrine functions integrate with behavior [29]. Moreover input into hypothalamic nuclei which directs hypothalamo-pituitary-adrenal and sympathoadrenal answers psychological stress includes cortical-limbic fibers [21].

Returning to Cro-Magnon image whose body uses modern human being it is necessary to keep in mind that this method of answering the danger corresponds to life conditions. During a clash with danger the activation of sympatical nervous system occurs and followed the increasing level of insulin and an arterial flow to muscles. Insulin helps all organism cells use sugar from blood which in other conditions can use just brain’s cells (they have no any sugar reserve as opposed to other sells of organism). This helps the organism to use the answer to danger: fight or flight. If this phase of stress answer is finished well, the next stage of stress answer would not come. But if danger is extremely large, that is the first reaction does not work, the second phase of the answer would switch on. It goes some later, because it takes some time to send the information using a chain of hormones. This delayed response is important because the consequence of the activity of the second phase of stress answer is extremely severe for the organism.

The purpose of the second phase of stress reaction connected with activation of a hypothalamo-pituitary-adrenal axis and surge of cortisol is to support the brain activation by the use of deactivating some basic mechanisms and decreasing the activity of other ones. An organism assesses the situation as extremely dangerous and directs all efforts to the maintenance of the executive organ. Cortisol has the receptors for all cells of an organism and it deactivates the more expensive systems: immune, recovery and sex because at the dangerous moment these functions could be disregarded. Then a suppression of insulin surge begins and as a consequence sugar can use just a brain. A brain needs oxygen, and then breathing becomes more intensive (we know how human experiencing, for example, deep grief breathes).

For Cro-Magnon this system was an effective one when he (she) met danger. This second phase of stress answer switched on vary rarely — when the resources of the organism ran out. The problem aroused, when stress became a mainly psychological one, and receiving the information a modern human being in case of need does not run and does not fight, but stands or seats on-site. His (her) muscles do not use adrenalin. Excess adrenalin with blood flow gets hypothalamic area and signals to a central system of stress regulation that there is an excess of this hormone. A system is adjusted thereby that an adrenalin excess in a blood flow is assessed as an extreme danger. It follows switching on the second phase of the stress answer though the circumstances do not need it. Moreover because an adrenalin excess does not disappear from blood this state becomes a chronicle one and this leads to all specter of basic diseases of adaptation. Then, lack of muscle’s moving under perception of emotional situation which is easy and it is possible to cope with it. A stress system adjusting to other conditions erroneously assesses situation as a critical one and an easy situation could follow chronic disorders. The type of this chronic disorder is predetermined with genetic peculiarities of an organism. The following changes are general for all people.

Chronic decrease immune system activity entails the following consequences. Per day about 6 million sells devising occur in a human body. These are predominantly skin and mucous cells which are exposed to the most intensive external influences and blood cells, inter alia lymphocytes actively meeting with infection and erythrocytes which have no any nuclei and can live not more than 3 months. It is known that for 1 million divisions a mistake occurs. Therefore about 6 potentially dangerous cells arise each day in each human being (this process is more intensive in childhood and lasts slower at an elderly age). In norm big immune cells — macrophages — are moving in a tissue and fag all cells which do not correspond to the criterion of norm, including potentially cancerous cells. While an immune system is in norm it protects the organism. In situations which organism erroneously (because of not adequate human behavior from the Cro-Magnon point of view) identifies as extremely dangerous, chronic immune system activity decreases results in cancerous diseases.

After a long period of suppression of immune system activity withdrawal from the stress doesn’t normalize its activity but results in its hyperactivity. If at this moment in environment there is pollen or domestic dust hyperactive immune system accepts this object as an aggressor and would begin to attack. This is the nature of connections between stress and allergic diseases.

A long period of chronic stress suppresses insulin surge. This results in distraction of beta cells of Langerhans’ insula synthesizing insulin and decrease of the tissue reaction to an insulin level. By-turn this results in continuation diabetes of the second type, or diabetes connected with stress.

The intensification of сells breathing in mitochondrions results in appearance of hyperoxide — oxygen receiving an additional electron. It participates in oxidative phosphorylation results in energetic molecules ATP. But if stress becomes a chronic process, hyperoxide is produced in excess, it goes out of cells and as a free radical could damage any molecules, the walls of the vessels. This process results in atherosclerotic plaques forming and later — ischemic heart disorder and its complication (stroke and infarction).

A cortisol promotes an emission of excessive quantity of hydrochloric acid in a gastrointestinal tract, and this result in ulcers of different parts of a gastrointestinal tract. Thereby we described all diseases, which are found on the first places in the list of population death. It is obvious that they origin as a result of inadequate behavior during experience of unpleasant events. This consequence is easily escaped using running or daily moving. It is a norm to move fast on foot 2 hours per day or do active exercises, not too long but more intensive.

Modern data show additional problems connected with the stress system. These data are received by American researchers during examination of children adopted in American families from Russian and Romanian orphanages. The peculiarities of these children were their lives in orphanages but not in families. Researchers tried to understand why the children adopted in families before 1 year and a half have fewer problems when they became teenagers, than children adopting after a year and a half. Children adopted after this period have negative behavior in teenager’s time obligatory. And this leads to returning children to orphanage or to changing an adoptive family [5].

Careful investigation of adopted children health in the process of their transfer from orphanage to families shows the changes in development of brain structures [12; 15; 16; 36; 38], which give the evidences of the brain structures’ maturation retardation as a consequence of stress experiences in the course of a year [23]. A researcher can prove that these changes predetermine the disruption of the adaptation process in adolescence [39].

The disruption of volitional functions and executive function are the most obvious problems of orphan children. They are observed both in a quiet state [17] and emotional excitation [33]. It was shown that the longer the child stays in orphanage the less he (she) is ready to control him (her) self [28]. More by token preschoolers who stayed in biological family very short periods of time and later lived just in orphanage did not show significant changes of executive functions even every other year of living in adoptive family [8]. Maybe it is connected with negative influence of stress on the later maturated prefrontal areas of cerebral cortex which are very sensitive to negative influences in early period of development.

Deterioration of a prefrontal cortex work was shown using different methods [7; 36; 40].

Deterioration of inhibiting control function is particularly evident in preschoolers and significantly depends on the time of acceptance in the family: the earlier the child was adopted into a family the better his (her) inhibiting functions are [24]. Exactly inhibiting functions insure the subsequent effective social communication and volitional control of behavior.

Between the biological courses of brain executive functions breaking could be the state of the systems respondent for the stress reaction. In the researches carried out on rats it was shown that in the first two weeks it was impossible to raise cortisol level on the stressors [32]. The explanation for this hyporesponsively is in the fact that the organism uses this mechanism for safeguard of the developing brain from the potentially dangerous influences of high cortisol level [13; 37].

Moreover, normal variation maternity of rats influence the forming of stress system [27]. Comparison of the females who groom and link their offspring carefully with those who do these rarely and not so carefully showed that later the first offspring fearlessly investigate the environment and better experience stress situation than the second one [26].

This differences are fixed on the molecular level. It was shown that licking and grooming reduce methylation of hippocampal GR genes [41]. GR genes determine how many hippocampal glucocorticoid receptors an animal will have [18]. Methylation is the basic mechanism genes closing.

Good maternity results in the larger quantity active receptors will reserve by the period when the organism goes out of the hyporeactivity to stress and is reluctant to answer to stressor in an adult way. Therefore, this organism will be more effective in the adult period [27]. This epigenetic effect potentially is irreversible [41].

The same phenomena were shown for the infants. After birth a cortisol level decreases in the case of an effective care of a child [22] and forming a safe attachment [42]. In unsafe relationships the stressors could excite increasing burst of cortisol, showing the activation of hypothalamo-pituitary-adrenal axis. This hyperactivation on stress in children lasts till adolescence. Along with the psychosocial process connected with a transition of a child to adolescent period the biological processes connected with a burst of sex hormones change neurobiology of a stress along the line of adult answer [35].

Thus the adults could prevent a cortisol growth in a child even in the stress situations. Responsible parenthood permits children to call for help expressing the negative emotions without endocrine system of stress regulation changing. On the contrary inadequate parenthood could be a source of danger and leads to changing a child’s stress reaction system [1; 30].

Hyporesponsivity on the stress events during a period of intensive growth is an evolutionary indicator presenting in many mammals and predetermining an effective brain work in an adult. Under a natural selection this mechanism promoted keeping of an individual with mature brain structures. The partial withdrawal of a human being from a natural selection and a lack of the system accounting of child development peculiarities lead to a situation when a high cortisol level beaks out a stress system in the first months of child’s life and delays the brain structures development and this process is lurked during a long time. It becomes evident just in adolescence when a burst of big quantity of sex hormones shows invalidity both of a stress system and self-behavior executive system. To restore these functions at that moment is not possible but any prophylaxis in parents training to early communication with child could be effective.

A changing of a stress system reaction could influence an effectiveness of any adaptation processes in future, it influences both physiological child growth and cognitive functions and in the final analysis of health. Therefore the parents’ behavior in this case predetermines a child’s health by way of forming a capacity to govern self-behavior.

Evolutionary roots of a modern sickness rate is assumed to trace both the peculiarity of stress system work because of discrepancy of the modern human behavior to body of Cro-Magnon and the specificity of a nutrition system.

An irregularity of food appearance in a time of our ancestries led to rigid mechanism of alimentary system which sent a scarce resource to a saving system. It did not use from this fat sources till the problem of survival emerged. Fat cells did not die as opposed to nervous ones. People prefer to choose freely a fat food. The fat human resources are never used if human eats irregularly. This indicates that a child overeating leads to an obesity which would accompany a human being during all his (her) lifespan.

Eating must depend on age. A human being must eat more fat and plentiful food in early childhood and less fattening food after 40—50 year old because food consumption needs to depend on a human activity. In the process of eating a human being need to assure his (her) organism that there is a lot of food (just in this case it would use fat). For this he (she) needs to eat small portions but sufficiently often (4—5 times per day). An infrequent and plentiful eating would lead to obesity, sufficiently often but small quantity — to adequate expenditure of eaten food.

So, analyzing human evolution it is possible to explain a lot of diseases by behavior which is inadequate to physiological mechanism. A behavior changing is assumed to link up with motivation and volitional mechanisms. Now the adaptation diseases sometimes are named as diseases of the disorder of a wish control: it is necessary to move, to restrict hem (her) self in food, to control emotions for well-being. And there the psychophysiology of a stress is finished and a new science begins — health psychology, whose purposes are to create a base for changing of a human behavior along the line of wish control.
