Аннотация. Рассматриваются психологические особенности стоматологических больных с заболеванием пародонта. Представлено описание пациентов с положительной и отрицательной динамикой лечения. Цель: выявление закономерностей между личностными особенностями пациентов и выполнением рекомендаций врача-стоматолога. Методы: обследовано 20 мужчин и 25 женщин, средний возраст пациентов составил 43 года. Использовались психодиагнос-тические методики: 1) Многомерный опросник по здоровью; 2) тест «Диагностика межличностных отношений»; 3) Характерологический опросник Леонгарда — Шмишека; 4) Индивидуально-типологический опросник Собчик Л.Н.; 5) Анкета самооценки состояния; 6) Личностный опросник Бехтеревского института. Измерялись стоматологические индексы здоровья полости рта: 1) CPITN; 2) PMA; 3) Saxer & Muhlemann; 4) Silness & Loe; 5) OHI-S. Результаты: По результатам двух измерений стоматологических индексов выявлено 3 группы пациентов: 1) с хорошей гигиеной полости рта, 2) с плохой гигиеной и положительной динамикой (улучшение индексов стоматологического здоровья), 3) с плохой гигиеной и отрицательной динамикой (ухудшение индексов). Установлена взаимосвязь выполнения рекомендаций врача-стоматолога с личностными особенностями пациентов. Обсуждение. Большинство пациентов старались выполнять рекомендации врача по уходу за полостью рта, о чем свидетельствует снижение индексов стоматологического здоровья на втором осмотре. Выявлены различия личностных особенностей пациентов, выполнявших и не выполнявших рекомендации, а также обнаружены гендерные различия в группах с улучшением и ухудшением индексов.

Актуальность

За последнее десятилетие в процессе социально-экономических реформ в стоматологии сформировались устойчивые рыночные отношения. В стоматологических лечебно-профилактических учреждениях развернулась борьба за пациента, его комфорт и удобство [11]. Все чаще пациенты стали обращаться за профессиональной помощью в платные стоматологические клиники. По статистике, только 20 млн. человек обращаются в государственные медицинские учреждения; в свою очередь, платные клиники принимают около 300 млн. человек в год [4].

Основные преимущества частных стоматологических клиник заключаются в качестве предоставляемых услуг и использовании новейших медицинских технологий, которые бюджетной медицине зачастую недоступны. Еще одним преимуществом является квалификация персонала: частные клиники живут за счет прибыли, которая зависит от количества пациентов. Если клиента не устроят результаты консультации или лечения, если ему не понравится отношение доктора и всего персонала в целом, то он просто уйдет к конкурентам [Там же].

Такое понятие, как «комплаентность», стало широко применяться врачами в основном при обсуждении лекарственной терапии. Синонимами этого термина являются «согласие пациента следовать рекомендациям врача», «приверженность терапии», «информированное согласие» [7]. Но редко уделяется внимание личностным особенностям пациентов, их ценностным ориентациям, образу жизни и социальному статусу. Ежегодно из-за последствий низкой комплаентности, по данным американских медиков, погибает 125.000 пациентов с различными заболеваниями. Около 10% всех случаев госпитализации связано с несоблюдением указаний лечащего врача. Значительная доля расходов, связанных с лечением в стационарах, также обусловлена низкой комплаентностью. Отсутствие данных об изучении комплаентности в стоматологической практике можно интерпретировать как неразработанность этой проблемы или как показатель низкого интереса к этому вопросу со стороны врачей стоматологического профиля [10].

Установление конструктивных взаимоотношений в системе «врач — пациент» оказывает влияние на уровень комплаентности [24]. В свою очередь, приверженность лечению проявляется в трех составляющих:

1)

своевременность приема пациентом лекарственного средства;

2)

прием лекарственного средства в назначенной дозе;

3)

соблюдение рекомендаций по питанию и образу жизни [9].

Заболевания пародонта являются самыми распространенными хроническими заболеваниями во всем мире. Более 90% населения имеют заболевания зубов и полости рта [20]. Тратятся миллиарды долларов для устранения последствий кариеса — воспаления и боли [19].

Выделяется несколько факторов, связанных с выполнением рекомендаций врача-стоматолога. Например, страх, волнение и напряжение перед посещением стоматолога испытывают от 52 до 85 % пациентов, около 5% испытывают сильнейший страх при одном упоминании о стоматологе [5; 12; 14; 15; 17; 25]. По мнению Анисимовой Е.Н., стойкая боязнь, доводящая пациента до панического страха, может формироваться после полученной психологической травмы в детстве, а также вследствие некорректного поведения врача или сильной боли.

Еще один важный фактор в повышении комплаентности — это психологические особенности пациента. Например, в зависимости от возраста, характера, личностных установок, отношения к болезни пациенты по-разному будут следить за своим здоровьем, выполнять рекомендации врача [6; 7; 22].

Индивидуальный подход к пациенту, с обеспечением его комфорта и безопасности на приеме, является ключом успешной работы [1; 13; 18]. Врачи признают важность построения доверительных отношений с пациентом и необходимость индивидуального подхода в выборе стратегии и разработки индивидуального плана лечения для разных типов стоматологических пациентов [3; 7].

Целью нашего исследования было установление закономерностей между личностными особенностями стоматологических пациентов и выполнением рекомендаций врача-стоматолога.

Материалы и методы исследования

Обследовано 45 человек (25 женщин, 20 мужчин) в возрасте от 19 до 67 лет. Использовались психодиагностические методики: 1) Многомерный опросник по здоровью (Мultidimensional Health Locus of Control Scales; Kenneth A. Wallson); 2) тест «Диагностика межличностных отношений» (Т. Лири); 3) Характерологический опросник Леонгарда — Шмишека (взрослый вариант); 4) Индивидуально-типологический опросник Собчик Л.Н. (ИТО); 5) Анкета самооценки состояния Военно-медицинской академии им. С.М. Кирова (АСС); 6) Личностный опросник Бехтеревского института (ЛОБИ). Измерялись стоматологические индексы здоровья полости рта: 1) CPITN; 2) PMA; 3) Saxer & Muhlemann; 4) Silness & Loe; 5) OHI-S. По всем стоматологическим шкалам использовалась общепринятая градация: 0 баллов — лечение не требуется; 1 балл — необходимо обучение индивидуальной гигиене полости рта и контроль гигиенического состояния пародонта; 2 балла — требуется профессиональная гигиена полости рта; 3, 4 балла — необходимо комплексное лечение заболеваний пародонта [2].

Процедура исследования

Все пациенты были осмотрены врачом-стоматологом, каждому были выставлены объективные оценки состояния зубов и полости рта с использованием стоматологических индексов. После этого с пациентом проводилась беседа, где врач давал рекомендации по правильному уходу за полостью рта, разъяснял необходимость соблюдений этих рекомендаций для улучшения здоровья пародонта. После этого назначалась повторная встреча для осмотра через 2 недели.

Результаты исследования

В результате исследования были получены стоматологические индексы при первом и повторном осмотрах, отражающие качество выполнения пациентом врачебных рекомендаций и динамику состояния пародонта через 2 недели. Полученные данные представлены в таблице 1.

После изучения стоматологических индексов пациентов на первом и втором осмотрах можно сделать выводы о том, что и для группы мужчин, и для группы женщин максимальное значение составляет не более 1,3 балла. Это означает, что у большинства испытуемых объективное состояние полости рта было хорошее. По большинству индексов значение менее 1 балла свидетельствует о том, что пациенты тщательно ухаживают за полостью рта. Процентное соотношение коэффициентов стоматологических индексов на первом и втором осмотрах представлено в таблице 2.

У исследуемых пациентов стоматологические индексы, измеряющие потребность в лечении пародонта, кровоточивости и воспаления десен, соответствовали норме, и также в пределах нормативных значений были индексы, оценивающие зубной налет.

Согласно таблице 2, состояние пародонта пациентов улучшилось ко второму посещению врача-стоматолога, и это может быть связано с тем, что пациенты в течение двух недель старались выполнять рекомендации по гигиене, полученные на первом приеме у врача-стоматолога.

Был проведен анализ личностных особенностей пациентов с положительной и отрицательной динамикой состояния полости рта у женщин и мужчин. В группу 1 (положительная динамика) попали пациенты, чьи стоматологические индексы снизились ко второму осмотру у врача-стоматолога. В группу 2 (отрицательная динамика) попали пациенты, у которых ко второму посещению индексы увеличились. Сравнительный анализ двух групп пациентов-женщин представлен в таблице 3.

По результатам сравнительного анализа установлено, что у пациенток с положительной динамикой выше внутренний локус контроля, т.е. в уходе за своим здоровьем они полагаются на себя. В обеих группах женщин преобладает альтруистический тип межличностных отношений. Также у них превалирует эмотивный тип характера, однако у пациенток группы 1 чаще встречается застревающий тип, а у группы 2 — зависимый. Пациентки с улучшением состояния полости рта более интровертированны, а у пациенток с ухудшением преобладает сензитивность. В обеих группах доминирует гармоничный тип отношения к болезни, однако мы обнаружили у группы 1 статистически значимое преобладание анозогнозического типа отношения к болезни.

Сравнительный анализ результатов пациентов-мужчин в группах с положительной и отрицательной динамикой представлен в таблице 4.

В результате проведенного анализа установлено, что пациенты-мужчины группы 1 обладают более высоким внутренним локусом контроля, по сравнению с пациентами группы 2. Для группы 1 характерен авторитарный тип межличностных отношений, а для группы 2 — агрессивный тип отношений. Это означает, что пациенты, которые тщательно выполняют рекомендации врача, более самостоятельны, активны в достижении целей, а пациенты с отрицательной динамикой — недоверчивые и враждебные по отношению к другим людям. Пациенты с отрицательной динамикой обладают застревающим типом характера, концентрируются на собственных переживаниях, обладают мнительностью и подозрительностью. А пациенты с положительной динамикой обладают демонстративным типом, т.е. для них характерны эмоциональная живость, подвижность, они стремятся быть в центре внимания. Пациенты обеих групп склонны к спонтанности, необдуманности в словах или действиях. Примечательно, что пациенты-мужчины группы 1, так же, как и пациенты-женщины группы 1, характеризуются анозогнозическим или, как еще его называют, эйфорическим отношением к болезни, т.е. такие пациенты сохраняют оптимизм в лечении, надеются, что проблемы со здоровьем несущественны и легко излечимы. А пациенты группы 2 обладают неврастеническим типом отношения к болезни, т.е. они нетерпимы к боли, не умеют терпеть или ждать, пока пройдут неприятные ощущения.

Выводы

1.

Большинство обследованных пациентов после проведенной специализированной беседы врача старались соблюдать рекомендации, полученные на первом приеме, и у большинства состояние пародонта объективно улучшилось ко второму посещению врача-стоматолога.

2.

По степени комплаентности пациенты разделились на 2 группы: пациенты, выполняющие и не выполняющие рекомендации врача, и между этими группами выявлены различия в личностных особенностях пациентов.

3.

Пациентки, приверженные выполнению рекомендаций, более интровертированны, «застреваемы» и исполнительны. Пациентки с отрицательной динамикой эмоционально более неуравновешенны, ранимы, не уверены в себе, зависимы от окружающих.

4.

Пациенты мужского пола с положительной динамикой полагаются в отношении здоровья на себя, более оптимистичны, целеустремленны и независимы, стремятся производить благоприятное впечатление. Пациенты-мужчины с отрицательной динамикой более недоверчивы, агрессивны и замкнуты, боязливы и нетерпимы к боли, плохо переносят лечебные процедуры.

Заключение

Таким образом, выявлены статистически значимые различия личностных особенностей у пациентов, выполняющих рекомендации врача и не выполняющих рекомендации. Исследование показало, что пациенты действительно выполняют назначения врача по-разному, в зависимости от личностных особенностей. На основании полученных в исследовании данных и при дальнейшем изучении пациентов необходим поиск более точных критериев, позволяющих определить, какие личностные характеристики взаимосвязаны с выполнением рекомендаций врача-стоматолога. Дальнейшие исследования позволят нам разработать способ эффективной диагностики пациентов и разделения их на группы для более точного подбора тактики и стратегии ведения пациента врачом-стоматологом.

Abstract. The article deals with the psychological peculiarities of dental patients with a periodontal disease. It gives the description of patients with positive and negative dynamics of treatment The goal: to reveal the regularities between personal peculiarities of patients and following the recommendations of a dentist. Methods: we have examined 20 men and 25 women; mean age of patients is 43. We have used psychodiagnostical techniques: 1) Multidimensional Health Locus of Control Scales; 2) Diagnostics of Interpersonal Relations Test; 3) Leonhard-Schmieschek’s Characterological Inventory; 4) L.N. Sobchik’s Individual Typological Inventory; 5) Self-Assessment Inventory; 6) The Bekhterev Institute Personality Inventory. We have measured the dental indices of oral cavity health: 1) CPITN; 2) PMA; 3) Saxer & Muhlemann; 4) Silness & Loe; 5) OHI-S. Results. The results of two measurements of dental indices have enabled to reveal 3 groups of patients: 1 — with good hygiene of an oral cavity, 2 — with bad hygiene and positive dynamics (better indices of dental health), 3 — with bad hygiene and negative dynamics (worse indices). We have established the interrelation between following the recommendations of a dentist and personal peculiarities of patients. Discussion. Most patients tried to follow the doctor’s recommendations on the care for an oral cavity; this is confirmed by lower indices of dental health during the second examination. We have revealed personal peculiarities of patients who followed and did not follow the recommendations; besides, we have identified gender differences in the groups with better and worse indices.

Relevance

For a recent decade, dentistry has seen the establishment of sustainable market relations during socio-economic reforms. Dental establishments began to struggle for a patient, his comfort and convenience [11]. Patients began to address more often to commercial dental clinics for professional aid. Statistics show that only 20 million people address to state medical establishments; commercial clinics, in turn, accept about 300 million people per year [4].

The main advantages of private dental clinics are the quality of services and the use of latest medical technologies, which budgetary medicine often cannot afford. Another advantage is the qualification of personnel: private clinics live off profit, which depends on the quality of patients. If a client is not satisfied with the results of an advice or treatment or if he does not like the attitude of a doctor or all the personnel in general, he will just go to competitors [Ibid].

The doctors began to use widely such notion as "compliancy" mostly in discussing medical therapy. The synonyms for this term are "patient’s consent to follow the recommendations of a doctor", "adherence", "informed consent" [7]. However, they rarely pay attention to personal peculiarities of patients, their value orientations, lifestyle, and social status. American doctors report that 125 000 patients with various diseases die every year because of low compliance. About 10% of all the cases of stationary admission are associated with non-compliance with the doctor’s instructions. A significant part of expenses related to in-patient treatment is also conditioned by low compliance. The lack of data on studying compliance in dental practice can be interpreted as underdevelopment of this issue or as an indicator of poor interest to this issue from dentists [10].

The establishment of constructive interactions in the "doctor — patient" system influences the level of compliance [24]. In its turn, adherence is manifested in three components:

Timely drug administration by a patent;

2)

Drug administration in the assigned doze;

3)

Following the recommendations on nutrition and lifestyle [9].

Periodontal diseases are the most widespread chronic diseases all over the world. More than 90% of population has dental diseases and diseases of an oral cavity [20]. Billions of dollars are spent to remove the consequences of caries: inflammation and pain [19].

There are several factors associated with following the recommendations of a dentist. Thus, from 52 to 85% of patients experience fear, anxiety and tension when they visit a dentist, about 5% experience the strongest fear at the bare mention of a dentist [5; 12; 14; 15; 17; 25]. Anisimova E.N. assumes that persistent fear leading a patient to scare can form after a psychological trauma in childhood or incorrect behavior of a doctor or strong pain.

Another important factor in the increase of compliance is psychological peculiarities of a patient. Thus, patients will care for their health and follow the recommendations of a doctor in a different way depending on age, temper, personal mindsets, and attitude to a disease [6; 7; 22].

An individual approach to a patient and providing his comfort and safety during his visit are the key to a successful work [1; 13; 18]. Doctors understand the importance of building trust-based relations with a patient and necessity of using an individual approach in the choice of a strategy and elaboration of an individual plan of treatment for various types of dental patients [3; 7].

The goal of our study has been to establish the regularities between personal peculiarities of dental patients and following the recommendations of a dentist.

Materials and methods

The study has involved 45 persons (25 women, 20 men) aged from 19 to 67. We have used psychodiagnostical techniques: 1) Multidimensional Health Locus of Control Scales (Kenneth A. Wallson); 2) Diagnostics of Interpersonal Relations Test (Т. Liri); 3) Leonhard-Schmieschek Characterological Inventory (adult variant); 4) L.N. Sobchik’s Individual Typological Inventory (ITI); 5) Self-Assessment Inventory of the Military Medical Academy named after S.M. Kirov (SAI); 6) The Bekhterev Institute Personality Questionnaire (BIPQ). We have measured the dental indices of oral cavity health: 1) CPITN; 2) PMA; 3) Saxer & Muhlemann; 4) Silness & Loe; 5) OHI-S. We have used commonly accepted gradation throughout all the dental scales: 0 points — no treatment needed; 1 point — teaching individual hygiene of an oral cavity and control of a hygienic state of periodontium needed; 2 points — professional hygiene of a n oral cavity needed; 3, 4 points — complex treatment of periodontal diseases needed [2].

Procedure of study

A dentist examined all the patients and gave objective assessments of state of teeth and an oral cavity to each patient using dental indices. After that, he had an interview with a patient, gave recommendations on proper care for an oral cavity, and explained the necessity to follow these recommendations to improve the health of periodontium. Then, he appointed the second visit for examination in 2 weeks.

Research results

The study has enabled to obtain dental indices at the first and second examinations reflecting the quality of following the doctor’s recommendations by a patient and dynamics of a state of periodontium in 2 weeks. The results are represented in Table 1.

Having studied the dental indices of patients at the first and second examination, we can conclude that the maximal value is no more than 1.3 points for both male and female groups. It means that most patients objectively have a good state of an oral cavity. The value less than 1 point by most indices show that patients care thoroughly for an oral cavity. The percentage ratio of the coefficients of dental indices at the first and second examinations is given in Table 2.

The dental indices measuring the need for treating periodontium, gingival hemorrhage and inflammation in patients under study have corresponded to a norm; the indices evaluating dental plaque have not exceeded normative values too.

As shown by Table 2, the state of periodontium has improved by the second visit of a dentist; this can be associated with patients trying to follow the recommendations on hygiene within two weeks received at the first visit of a dentist.

We have analyzed personal peculiarities of patients with positive and negative dynamics of a state of an oral cavity in women and men. Group 1 (positive dynamics) involves patients whose dental indices have decreased by the second examination by a dentist. Group 2 (negative dynamics) involves patients whose indices have increased by the second visit. The comparative analysis of two groups of female patients is represented in Table 3.

The comparative analysis has established that female patients with positive dynamics have a higher internal locus of control, i.e. they rely on themselves in care for their health. Both groups of women have mostly an altruistic type of interpersonal relations. Besides, they have mostly an emotive type of character; however, the patients from group 1 more often have a sticking type, while group 2 — a dependent type. Female patients with better state of an oral cavity are more introverted, while female patients with worse state are more sensitive. Both groups have mostly harmonious type of an attitude to a disease; however, we have found statistically significant prevalence of an anosognosic type of an attitude to a disease in group 1.

The comparative analysis of the results of male patients in groups with positive and negative dynamics is represented in table 4.

The analysis has enabled to establish that male patients from group 1 have higher internal locus of control compared to patients from group 2. Group 1 has an authoritarian type of interpersonal relations, while group 2 — an aggressive type of relations; it means that patients who follow the doctor’s recommendations thoroughly are more independent and active in achieving goals, while patients with negative dynamics are distrustful and hostile to other people. Patients with negative dynamics have a sticking type of character and focus on their own experiences; they are hypochondriac and suspicious. On the contrary, patients with positive dynamics have a demonstrative type, i.e. they have vivid emotions; they are mobile and try to be the focus of attention. Patients from both groups are prone to spontaneity and hastiness in words or actions. Interestingly, male patients from group 1 as well as female patients from group 1 have anosognosic or, in other words, euphoric attitude to a disease, i.e. such patients stay optimistic about treatment and hope that problems with health are insufficient and easily curable. Patients with group 2 have a neurasthenic type of an attitude to a disease, i.e. they are intolerant to pain, and they cannot endure or wait until unpleasant feelings pass.

Findings

1.

Most patients under examination tried to follow the recommendations received at the first visit after a specialized interview by a doctor; therefore, in most patients, the state of periodontium objectively became better by the second visit of a dentist.

Patients have been subdivided into two groups by the degree of compliance: patients who follow or do not follow the doctor’s recommendations; we have revealed differences in personal peculiarities of patients between these groups.

Female patients who adhere to recommendations are more introverted, "sticky", and diligent. Female patients with negative dynamics are more emotionally unbalanced, vulnerable, unsure of themselves, and dependent on others.

Male patients with positive dynamics rely on themselves with respect to their health, they are more optimistic, goal-oriented, and independent, and they strive to produce a favorable impression. Male patients with negative dynamics are more distrustful, aggressive, and closed, they are fearful and intolerant to pain, and they poorly endure treatment procedures.

Conclusion

Thus, we have revealed statistically significant differences of personal peculiarities in patients who follow or do not follow the recommendations of a doctor. The study has shown that patients actually follow the prescriptions of a doctor in a different way depending on personal peculiarities. We need to look for more accurate criteria based on the results obtained during the study and in later studying of patients to define what personal characteristics are interrelated to following the recommendations of a dentist. Further studies will allow us to elaborate a way to diagnose patients efficiently and divide them into groups for more accurate choice of tactics and strategy of treating a patient by a dentist.
