Отражение исторических событий в клинической картине параноидной шизофрении

Аннотация. В настоящее время — время доказательной науки и высоких технологий, формирование симптомов шизофрении рассматривается с точки зрения биологической парадигмы — как прямое следствие гиперпродукции моноаминов, преимущественно дофамина. Работы основоположников учения о шизофрении, подчеркивавших полиэтиологичность расстройства, чаще всего упоминаются в контексте истории психиатрии. Тем не менее, все большее количество исследований подчеркивает определяющее значение социокультуральных факторов в формировании симптомов, содержание которых отражает особенности исторического времени конкретного сообщества. Статья демонстрирует как универсальные, так и индивидуально-значимые влияния времени на содержание переживаний пациентов с психическими расстройствами. Авторы утверждают, что содержание симптомов шизофрении отражает своего рода «экстракт» определенного исторического времени, состоящий из причудливого набора событий и влиятельных персонажей, вплетенных в канву личностного прошлого и настоящего каждого пациента. Кроме подобной «реальной» клинической картины с отражением событий, определяющих общественно-политическую жизнь общества, авторы рассматривают механизмы формирования «нереальной» фабулы галлюцинаторно-бредовых переживаний. В этих случаях основными героями происходящего становятся либо мифические персонажи, такие как джинны, либо устрашающие герои современных фильмов ужасов — вампиры и зомби. Авторы статьи обращают внимание читателя на парадокс между логичностью отражения событий в бредовой интерпретации и паралогичностью их индивидуального восприятия. Клинические примеры, использованные в статье, предлагают читателю проследить включение одних событий и игнорирование других в процессе формирования содержания галлюцинаций и бреда.

Вопрос об универсальности формы и специфичности содержания симптомов шизофрении продолжает оставаться актуальным, хотя и рассматривался неоднократно [31; 32; 33; 36; 37; 44; 46]. Утверждение о том, что время накладывает значимый отпечаток на содержание психопатологической симптоматики психических расстройств, представляется банальным. Тем не менее, работ, описывающих причины отражения одних событий и игнорирования других, не менее значимых с точки зрения «времени», не так много [30; 35; 38; 47; 51]. Под «временем» здесь и далее мы понимаем концепцию «исторического времени». Историческое время — это время, предстающее в качестве атрибутивного свойства истории, определяющего ее как выражение прошлого, как процесс изменения, преобразования и развития человечества, общества, отдельных народов, стран и государств [21]. Время позволяет осуществить хронологию и периодизацию исторического процесса, а также отразить направленность исторического движения в определениях прошлого, настоящего и будущего, изобразить историю как последовательную смену качественно различных состояний экономического, социального, политического и духовного развития общества, образующих исторические эпохи [Там же].

Вопрос содержания психопатологических переживаний и отражения в них исторических событий тесно связан с изучением этиологии и патогенеза шизофрении. В процессе развития учения формулировались многочисленные взгляды: от теории чистой эндогенной этиологии [20] до отрицания существования шизофрении как нозологической единицы [50]. Между двумя крайними полюсами континуума находится представление о полиэтиологичности расстройства и множественном экзогенном влиянии на формирование симптомов.

Сложное взаимодействие экзо- и эндогенных факторов в формировании психоза подчеркивалось классиками психиатрии. Основываясь на нозологизме Э. Крепелина, Э. Блейлер в 1906 году изучал формирование бредовых идей под воздействием разнообразных экзогенных факторов [2]. Сходные взгляды обнаруживаются в работах Э. Бонгеффера 1908—1912 годов, связывавших влияние многочисленных экзогенных факторов с ограниченным количеством эндогенных реакций. Каннабих Ю.В. подчеркивал влияние преморбидных особенностей в отношении формирования продуктивной симптоматики психоза [7]. В 1910 году К.Г. Юнг и Э. Блейлер, основываясь на теории З. Фрейда, показали значение первоначальных аффективных переживаний в формировании содержания продуктивной симптоматики [Там же].

История развития науки знает множество примеров, когда непересекающиеся между собой теории становятся проекциями одного целого. В 1918 году Э.Р. Кречмер в работе «О сенситивном бреде отношения» объединил две различные точки зрения, а в 1919 году К. Бирнбаум в «Построении психоза» показал механизм конструирования психоза в результате влияния комплекса разнообразных эндогенных, экзогенных, психических и элементарно-соматических, функциональных и органических факторов [1]. Сходные идеи представлены в трудах Гаупп (1909), Гохе (1912), Странского (1912), Иельгерсма (1912), Ясперса (1913), Леви-Валенси (1927) и других исследователей.

В современных работах, таких как диссертация С.В. Назимовой «Особенности психопатологии и клиники приступообразной шизофрении, формирующейся в условиях воздействия экзогенных факторов», подтверждается влияние нескольких переменных в формировании психозов и, в частности, шизофрении [17].

Для объяснения комплексного взаимодействия экзогенных и эндогенных факторов в формировании шизофрении можно провести аналогию с концепцией стресса Г. Селье [22], определяющего стресс как неспецифическую реакцию на воздействие любых стимулов. Можно предположить, что именно по типу стандартной неспецифической реакции проявляется форма параноидной шизофрении, а культуральные различия формируются под воздействием исторического времени, места проживания, культуры, принадлежности к возрастной группе и других, не менее значимых переменных.

Очевидные различия в культурных ценностях, поведении, особенностях восприятия мира у разных народов предопределили интерес к культурально обусловленным особенностям психических расстройств. Отражение исторического времени в психопатологической продукции подчиняется определенным закономерностям, которые прослеживаются в самых ранних работах С.С. Корсакова, В.П. Сербского, В. Гельпаха и других. В донозологический период психопатологическая симптоматика была наполнена страхами, обусловленными невозможностью объяснить природные явления. Именно в то время С.С. Корсаков (1901) подчёркивал, что существенное значение в возникновении психозов имеет воспитание детей в обстановке религиозного мистицизма и суеверия. Он утверждал, что религия не влияет на душевные заболевания, но религиозный фанатизм и суеверия могут выступать в качестве триггеров психических заболеваний [9]. Категория «религиозное помешательство» (paranoia religiosa) было описано В.П. Сербским в 1906 году. Синдром включал зрительные и слуховые галлюцинации религиозного содержания и религиозный бред о божественном призвании. Мужчины чаще отождествляли себя с пророками, женщины — с Христовыми невестами, поведение соответствовало фабуле бреда [24]. В 1908 году В. Гельпах писал, что религия почти всегда имеет болезненную основу, связывая, таким образом, религиозные переживания с симптомами расстройства [5]. В 1911 году Э. Блейлер утверждал, что религиозное помешательство может встречаться при любой психической патологии, подчеркивая тем самым нозологическую неспецифичность синдрома [3]. В 1920 году Э. Крепелин писал о том, что при чрезмерной религиозности у пациентов формируется религиозный бред преимущественно экспансивного характера, например, касающийся связи с Богом и мессианства [11]. В 1927 году К. Шнайдер весьма категорично определил чрезмерную религиозность в качестве «психического отклонения», отметив универсальность феномена и подчеркнув, что одной из причин может быть особая личность, в частности, истерическая [25]. Подобные наблюдения описаны Р. Крафтом-Эбингом в 1890 году [10] и K. Ясперсом в 1931 году [26]. В сравнительно недавней работе О.А. Панченко, И.И. Кутько, В.Н. Березовского [18] также отмечено, что содержанием психопатологических переживаний истерических пациентов того времени были галлюцинации и бредовые идеи религиозного содержания [Там же].

По мере накопления обширных знаний в сфере социокультуральных особенностей психических расстройств транс-, меж- и этнокультуральные исследования становились все более актуальными. Так, в 1967 году P. Yap одним из первых выделил понятие «культурально-специфический синдром» — термин, применяющийся при обозначении психического нездоровья, специфичного для определенного региона. В 1985 году N. Wig вводит понятие «психокультуральных синдромов» [16].

Изыскания, посвященные поиску различий в симптомах психозов, в зависимости от места проживания, этнической принадлежности и возраста респондентов, остаются по-прежнему интересными. Полученные данные могут показаться неважными с точки зрения сиюминутной практической значимости, однако представляют несомненный интерес для понимания обнаженных психозом культуральных и иных различий.

Исследования культуральной специфичности болезненных переживаний имеют давнюю историю. Так, еще в 1904 году Э. Крепелин во время поездки на остров Яву установил, что у аборигенов при развитии dementia praecox аффективные, кататонические симптомы и слуховые галлюцинации встречаются реже, а спутанность сознания и «болтливое слабоумие» — чаще, чем у больных в Европе [40]. Типичная для жителей о. Маврикий клиническая картина психоза была описана A. Raman и H. Murphy (1971). Обнаружено, что степень возбуждения у жителей о. Маврикий была выше, а уровень дефицитарных симптомов ниже, чем у пациентов с шизофренией в Англии. Также удалось установить, что прогноз у пациентов с шизофренией и шизофреноформными расстройствами у жителей о. Маврикий благоприятнее по сравнению с Европой и Америкой [43]. Аналогичные исследования были сделаны W. Pfafflin, E. Wulff [53]. Они отметили редкость симптомов первого ранга при шизофрении в азиатских странах. Всеми исследователями были выделены особенности проявления заболеваний в различных культурных средах. M. Katz, K. Sandhorn и H. Gudeman (1970) обратили внимание на то, что у пациентов филиппинского или полинезийского происхождения наблюдались преимущественно симптомы возбуждения, в то время как у японцев преобладали проявления аутизма [39]. M. Opler (1956) и J. Singer (1956) изучали различия у американских граждан итальянского и ирландского происхождения, обнаружив значительно более высокий уровень воображения и двигательной активности у последних [49]. Ипохондрический фасад депрессии у жителей Северной Африки, большая выраженность расстройств сознания и кататонических синдромов в картине шизофрении описаны Wittkower E., Okashe A. [45; 52].

Описательный характер большинства ранних публикаций в области психиатрии был основан на наблюдении случайных клинических случаев. Проведение экспериментальных исследований, даже с участием добровольцев, невозможно по этическим причинам. Однако в тех случаях, когда жизнь сама предоставляет необходимую информацию, исследователи могут и должны подвергать данные тщательному изучению.

Одним из доказательств, влияния социокультуральных факторов на формирование галлюцинаторно-параноидной симптоматики представляются особенности переживаний, вызванных влиянием галлюциногенов в различных временных и культурных контекстах. Изучено, что на переживания, возникающие от приема галлюциногенов, оказывают влияние такие особенности, как система верований, мотивация, настроение, черты характера индивидуума, а также его физическое состояние [27; 28]. Доказано, что особенности культуры играют решающую роль в формировании и, более того, даже в программировании субъективных ощущений индивидуума. Описана роль культуры в видениях, вызываемых под действием галлюциногена. Например, для удачи на охоте, используя свойства галлюциногенов, мужчины племени амахуака воссоздавали в видениях движения и действия различных животных. Оказалось, что они изучали особенности поведения животного, используя содержание галлюцинаций для последующего осознанного воспроизведения. Подобным образом поступали шаманы из разных племен [34].

Историческое время может быть рассмотрено как часть культуры. Значимые события изменяют содержание психопатологических переживаний, отражая происходящее в мире, актуальные проблемы и новые открытия [30], политическую обстановку, религиозные взгляды [47], преломляясь через призму психического расстройства.

Психореактивный эффект исторического времени отражен в следующем клиническом примере.

Пациент А., возраст 34 года. Расстройство дебютировало в 2001 году. За время болезни сформировались стойкие, систематизированные галлюцинаторно-параноидные переживания. В бредовой фабуле отмечались бредовые идеи отношения, воздействия, отравления, особых возможностей. Считал, что сестра подсыпает в еду фекалии, так как завидует ему из-за его особых способностей. Ощущал воздействие различных сил. Утверждал, что может управлять погодой, вызывать дождь или ветер, влиять на людей, особенно на их мысли. Клиническая картина оставалась стабильной в течение 13 лет, пока в 2014 году не стал утверждать, что именно он определяет события в мире. Был уверен, что именно он изменил политическую обстановку в США и Кыргызстане. Утверждал, что с помощью своих способностей вызвал ураган в 2008 году в США и тем самым дестабилизировал ситуацию в стране. Уверял, что именно он повлиял на президентские выборы в США. С 2015 года клиническая картина параноидной шизофрении становится все более политизированной. Считал, что согласовывал с В.В. Путиным проведение обеих революций в КР (и/б № 2650-315, 2014; и/б № 209, 2015).

Отражение политических событий в переживаниях пациентов хорошо знакомо врачам-психиатрам. Следует, тем не менее, отметить, что подобное отражение имеет свою систему — чем большее историческое значение приобретает событие в будущем, тем более вероятно его отражение в бредовой фабуле [48].

Содержание бреда отражает не только культуру определенного времени и значимые общественно-политические события, но и те особенности общественной жизни, которые обычно не освещаются прессой, а также господствующие страхи и суеверия, агрессивное возрождение религии, древних ритуалов и язычества. Бредовая фабула отражала как тему сифилиса в начале 1900-х годов, так и немецкий нацизм времен Второй мировой войны [30], преследование шпионами враждебных государств в 50-е годы прошлого века [Там же], слежку со стороны Комитета государственной безопасности Советского Союза [14], перестройку и бандитизм ранних 90-х, а также революции и межэтническое напряжение начала XXI века [13].

Ученый из Южной Кореи Kwangiel Kim (2001) провел ряд исследований, посвященных влиянию культуры на психотическую симптоматику. В своих статьях он приводит ряд наглядных примеров влияния политики на фабулу бреда у пациентов с параноидной шизофренией. Так, его пациенты утверждали, что за ними следят шпионы из Северной Кореи. Автор статьи предполагает, что в данном случае бред преследования отражал военную и политическую угрозу со стороны Северной Кореи того времени. В статье также говорится о том, что с 1960 по 1980 годы часто встречались пациенты с бредом, связанным со шпионажем коммунистов в Китае и Тайване. В последнее время «политический бред» встречается все меньше, и, по мнению автора, это связано со смягчением отношений между двумя державами [47].

Следующий клинический пример иллюстрирует вышеизложенное.

Пациент В., возраст 48 лет. Заболевание дебютировало в 1989 году. Сформировались стойкие, систематизированные бредовые идеи отношения, воздействия, ущерба. В 2009 году стали вплетаться новые события, бредовые идеи расширились за счет бреда особых возможностей, величия. Считал себя криминальным авторитетом, который пользовался особым уважением среди самых главных воров в законе. Утверждал, что постоянно посещает «сходки» и решает криминальные проблемы. Считал, что его преследовали агенты Совета национальной безопасности и милиция, везде видел заговоры, следы и доказательства преследования (и/б № 345, 2010).

Именно в это время в Кыргызской Республике отмечался расцвет криминала с переделом сфер влияния, чему немало способствовала нестабильная политическая обстановка с растущей неуверенностью в завтрашнем дне.

Следующие два примера из нашей практики иллюстрируют политический бред преследования.

Пациент С., возраст 24 года. Первый отчетливый психотический эпизод наблюдался в 2008 году. Первоначально в фабуле бреда были преимущественно бредовые идеи экспансивного реестра, пациент считал себя весьма значимой фигурой в бизнесе и политике, говорил, что общается телепатически с депутатами, в частности — с четой Бакиевых, строил планы по поднятию экономики. В 2009 году к фабуле расстройства присоединились бредовые идеи преследования по политическим мотивам, стал говорить, что его преследуют спецслужбы по заказу Жаныша Бакиева, стал замечать людей в костюмах, видел постоянно преследующие тонированные машины (и/б № 82, 2009).

Следует отметить, что к 2009 году очевидная слабость правительства Бакиева, наряду с неприкрытой коррупционной деятельностью членов его семьи, набирала в общественном дискурсе все большую популярность. Парадокс, характерный для отражения исторических событий в симптоматике параноидной шизофрении, заключается в логичном отражении наиболее значимых моментов при паралогичности их индивидуальной интерпретации.

Пациент Д., возраст 30 лет. Расстройство дебютировало в 2011 году. За 4 года сформировались систематизированные бредовые идеи. В бредовой фабуле идеи величия, особой значимости. Считал себя ценным сотрудником спецслужбы, утверждал, что из-за этого за ним следили, его разговоры прослушивали, вели записи, заполняли досье. Потом его стали посылать на задание по предотвращению покушения на президента. Постоянно говорил о государственном долге, о плане покушения и государственном перевороте, из-за чего якобы стал подвергаться политическому преследованию, говорил, что за ним скоро приедут сотрудники, что это проделки правительства. Содержанием бреда были экспансивные идеи в дебюте, но очень быстро присоединились идеи преследования, что связано с политической нестабильностью, недавними революциями (и/б № 3044/357, 2015).

Мы настаиваем на том, что фабула основных симптомов шизофрении всегда связана с реальностью. Тем не менее, кроме описанных выше примеров, все чаще в клинической практике стали встречаться «нереальные», фантастические, сказочные и религиозные персонажи.

Так, в последнее время все чаще в клинической картине пациентов кыргызской этнической принадлежности стали преобладать идеи воздействия со стороны сверхъестественных персонажей. Со слов одного из врачей Республиканского центра психического здоровья, «столько джиннов в отделении еще никогда не было». «Вселение джиннов» и их воздействие на мысли, чувства, а также подчинение воли пациента желаниям сверхъестественных сил, управление его/ее поступками чаще всего рационализируется как «испытание», результатом которого должны стать особые способности, такие как «кыргызчылык» или «көзү ачык». Объяснение особой популярности мифических персонажей в клинической картине параноидной шизофрении, казалось бы, лежит на поверхности. С конца 90-х годов прошлого века, на фоне всеобщего хаоса, политической и социальной нестабильности, крайней неуверенности в будущем, отмечался ренессанс традиционных духовных практик, включающих, в том числе, ритуалы изгнания джиннов, ответственных за злые мысли, желания и поступки. Ритуалы экзорцизма проводятся либо на святом месте целителем күүчү — специалистом по изгнанию джиннов, — либо в мечети. Обязательная часть ритуала — использование камчи (плетки), при помощи которой джинн (или джинны) изгоняются через тело пациента. Следует отметить, что состояния овладения рассматриваются в контексте не только параноидной шизофрении, но и  диссоциативных расстройств. В этих случаях джинн выступает либо как символическая репрезентация вытесненного материала, либо в качестве изолированной, плохой или неудобной части личности. Не случайно во время эпидемии конверсионных расстройств на Кавказе [8], в Северном Казахстане [15] и в других странах постсоветского пространства экзорцизм стал массовым лечебным мероприятием. В 2009 году священнослужителем и помощником главы Чеченской Республики Даудом Сельмурзаевым в Грозном был открыт Центр исламской медицины, причем наибольшей популярностью в Центре пользуется процедура изгнания джиннов.

Следующий клинический пример показывает отражение этнических особенностей в клинической картине и трансформацию бредовой симптоматики.

Пациент Е., возраст 33 года. Есть сведения, что его дедушка — «известный молдо в селе». Заболевание дебютировало в 2009 году. Нарушился ночной сон, перестал ухаживать за собой, стал отгороженным, злобным, конфликтным. В клинической картине наблюдались вербальные псевдогаллюцинации императивного и комментирующего характера. Высказывал бредовые идеи отношения к родным, идеи воздействия, стал говорить, что «это действие джиннов, которые вселились в него от неправильного образа жизни и от того, что его жена нечистая». В течение 2-х лет получал «специфическое лечение» у дедушки-молдо, который пророчил ему «просветление». Но так как состояние пациента ухудшалось, был дважды госпитализирован в острое психиатрическое отделение в 2011 году. После выписки стал посещать «святые места на юге республики», продолжал лечение у дедушки. В 2014 году клиническая картина изменилась, стал носить специфическую одежду, отпустил бороду, стал вести себя вызывающе, высокомерно. К бредовой фабуле присоединились бредовые идеи величия, особых возможностей. Утверждал, что теперь он пророк, просветленный, может лечить людей и читать их мысли, способен предсказывать будущее. Стал избивать жену, таким образом очищая ее от джиннов. Говорил о том, что из родственников тоже надо изгнать джиннов, как изгнали из него (и/б № 1209/190, 2016).

По мнению Marek Krzystanek [46], изменения религии в обществе влекут за собой изменения галлюцинаций и бреда религиозного содержания.

Бредовые идеи отражают основные религиозные изменения в обществе. Важно отметить, что после посещения святых мест наши пациенты стали раздавать и сжигать вещи, объясняя, что «из-за них дом нечистый и вещи осквернены, что так велел Бог, чтобы очистить свое окружение». Они высказывали бредовые идеи, утверждая, что «надо делиться с ближними, как в христианстве. Родители неправильно живут, в них джинны и шайтаны» (и/б № 180, 2013; и/б № 3110-364, 2013; и/б № 529-112, 2016; и/б № 1900/271, 2016).

В связи с неустойчивой позицией религии в стране, появлением новых религиозных направлений, а также с укреплением радикального ислама в содержании бреда стали смешиваться различные религиозные направления.

По мере все большего внедрения технологий в жизнь современного человека  содержание бреда стало включать компьютеры [51], интернет [38] и компьютерные игры [35]. Согласно Vaughan Bell (2005), инновации в технологии влияют не только на содержание бреда, они также влияют на его происхождение и формирование. Можно рассматривать формирование бреда, связанного с интернетом и новыми технологиями, как разновидность культурального бреда по DSM V или бреда воздействия [38]. Подобное мнение было высказано Vladimir Lerner, утверждавшим, что бред, связанный с интернетом, является не новой категорией бреда, а видоизмененным бредом преследования [41]. Понятно, что бредовые идеи, связанные с интернетом и компьютерными играми, чаще наблюдаются у молодых пациентов.

Пациент М., возраст 20 лет, дебют заболевания — в 2010 году. В бредовой фабуле отразились события из современных триллеров и интернет-историй. Стал говорить, что вокруг него мертвецы, зомби, описывал их страшный вид. Периодически чувствовал, что сам меняется, что в горле появляется обилие слюны и глаза становятся страшными. Утверждал, что они вокруг, преследуют его. Ощущал на себе их воздействие, чувствовал, как они меняют его мысли (и/б № 4862-445, 2012).

Характерной особенностью данного случая является молодой возраст и, что важнее, дебют заболевания, пришедшийся на период активного проката фильмов с сюжетом, отраженным в его фабуле. Большое количество интернет-игр, сюжетом которых часто являются апокалиптические истории или фантастические персонажи, такие как зомби, вампиры, орки, безусловно, повлияло на развитие содержания переживаний этого молодого человека.

 

Таким образом, вопрос о влиянии исторического времени на особенности содержания психопатологии при психических расстройствах возник с момента зарождения общества, в котором стали появляться психически больные люди. В период становления психиатрии как науки доминировали три фундаментальные теории этиопатогенеза психических болезней: эндогенная, экзогенная и полиэтиологическая. На современном этапе развития психиатрии достоверно доказано, что на формирование психопатологии оказывают влияние как эндогенные, так и экзогенные виды воздействия. В содержание психопатологии стала вплетаться трансформированная психозом информация о текущих событиях, информационных технологиях, революционных идеях. На каждом этапе развития общества в психопатологии отражались актуальные проблемы, открытия, значимые фигуры. Содержанием переживаний стали многообразные варианты трансформации галлюцинаторно-бредовых симптомов на основе первоначальных культурных ценностей, идеалов, личностных особенностей, характерных для данного индивида и данного исторического времени.

Abstract. In the present time — time of evidentiary science and high technology, formation of schizophrenia symptoms are considered as an direct sequence of monoamine hyperproduction, predominantly — dopamine. Works of founders studying schizophrenia, emphasizing polyetiology of the disorder, more often mentioned in the context of history of psychiatry. However, a growing number of researches emphasizes the crucial socio-cultural factors, in the process of forming symptoms, reflecting specifics of historical time in a particular community. The present article has a review format and shows universality of time influence on the content of patients’ experience with mental disorders. Psychopathological signs reflect general and significant phenomenon, the whole "extract" of a particular historical time, consisting of a bizarre set of events, influential characters twisted in an individual history of a patient. Except detailed "real" clinical picture reflecting socio-political events, authors consider formatting mechanisms of "unreal" content of hallucinatory-delusional symptoms. In such cases main heroes are mythological characters for example gins or intimidating heroes of modern movies like vampires and zombies. Authors of the article focus reader’s attention on paradox of logical reflection of events in the context of delusional symptoms and individual interpretation of delusions. In the present paper, researchers provide clinical cases showing how some events were included and others were ignored in the forming process of a content of hallucinations and delusions.

Neverthetless a question about the universality of form and specificity of content of schizophrenia symptoms have been considered numerous times, it is still relevant now [31; 32; 33; 36; 37; 44; 46]. It seems obvious that time has an effect on the content of psychopathological symptoms in mental disorders. Even though, there are only few works that describe reasons of reflection of some events and ignoring others, which are less important according to "time" [30; 35; 38; 47; 51]. In the present article, time is considered as "historical time". Historical time is a time represented as an attributive characteristic of history and definines it as an expression of the past, as the process of changing, transformation and development of humanity, society, specific communities, states and countries [21]. Time enables realization of chronology and periodization of historical process, it also reflects direction of historical motion in the frame of past, present and future. It shows history as a consistent changes of qualitatively different states of economical, social, political and spiritual development of a society, that form historical eras [Ibid].

The question about the content of psychopathological symptoms and reflection of historical events is closely related to the investigation of etiology and pathogenesis of schizophrenia. In the process of studying, numerous approaches were created: from the theory of endogenous etiology [20], to denial the existence of schizophrenia as a specific disorder [50]. Between these two contrasts there is an idea showing that schizophrenia is polyetiological disorder and it has numerous external influences on formation of symptoms.

Classical psychiatrists underlined complex interaction of exogenous and endogenous factors in psychosis formation. Based on nosology of Kraepelin, in 1906 Eugen Bleuler was studying formation of delusional ideas under the influence of various exogenous factors [2], similar ideas had been found in studies of Bonhoeffer in 1908—1912, connecting the influence of a large number of exogenous factors with a limited number of endogenous reactions. Kannabikh Y. emphasized the impact of premorbid characteristics on the formation of positive symptoms in psychosis [7]. In 1910, C.G. Jung and E. Bleuler, based on the theory of Sigmund Freud, showed the meaning of the original affective experiences in the content shaping of psychoproductive symptoms [Ibid].

The history of science development shows numerous examples when independent theories reflect the same idea. In 1918, E.R. Kretschmer in his work "About the sensitive delusion of relations" brought together two different points of view, and in 1919 K. Birnbaum in "Building psychosis" showed the construction of mechanism of psychosis as a result of influence of the complex variety of endogenous, exogenous, mental and elementary physical, functional and organic factors [1]. Similar ideas were found in works of Gaupp (1909), Gohe (1912), Stranskii (1912), Ielgersma (1912), Jaspers (1913), Levi-Valensi (1927).

In modern works, such as the thesis of S.V. Nazimova "Specifics of psychopathology and syndromes of paroxysmal schizophrenia, shaping under the impact of exogenous factors," confirmed the influence of several variables in the formation of psychoses and schizophrenia in particular [17].

To explain the complex interaction of exogenous and endogenous factors in the development of schizophrenia, it is possible to provide an analogy with the concept of stress by Selye H. [22], defining the stress as a nonspecific response on the impact of any stimuli. It can be assumed that according to the type of a standard non-specific reaction paranoid schizophrenia is generated, and cultural differences are shaped by historical time, place of living, culture, belonging to the age group and the other equally important variables.

Obvious differences in cultural values, behavior, perception of the world especially in different nations determined the interest in culturally conditioned peculiarities of mental disorders. Reflection of historical time in psychopathological production obey the certain laws, which can be followed in the earliest works of S.S. Korsakov, V.P. Serbsky, V. Gelpaha and others. In prenosological period, there were a lot of fears in psychopathological symptomatology caused by inability to explain natural phenomena. During that time, Korsakov (1901), emphasized that the significant meaning in the onset of psychoses is raising children in an atmosphere of religious mysticism and superstition. He argued that religion has no effect on mental disorders, but religious fanaticism and superstition can act as triggers of mental illness [9]. The category "paranoia religiosa" was described by V.P. Serbskii in 1906. The syndrome includes visual and auditory hallucinations with religious content and religious delusions of divine calling. It is more often when men identified themselves with prophets and women identified with Christ’s brides, in such cases behaviors were corresponded to delusional content [24]. In 1908, V. Gelpah wrote that almost almost always has unhealthy basis, thus, connecting religious experiences with the symptoms of the disorder [5]. In 1911, E. Bleuler argued that "paranoia religiosa" could be encountered in any mental disorders, emphasizing the nosological nonspecificity of the syndrome [3]. In 1920, Emil Kraepelin asserted that in patients the excessive religiosity is formed in a religious delusion mostly having an expansive character, such as connection with God and messianism [11]. In 1927, K. Schneider quite categorically identified the excessive religiosity as a "mental disorder", noting the universality of the phenomenon, and underlined that one of the reasons may be a special personality in particular-hysterical type [25]. Similar observations are described by R. Krafft-Ebing in 1890 [10] and K. Jaspers in 1931 [26]. In a relatively recent work of Panchenko O.A., Kut'ko I.I., Berezovsky V.N. [18] it was also mentioned that at that time the content of psychopathological experiences of hysterical patients included hallucinations and delusions with religious content [Ibid].

The more knowledge was accumulated in the field of socio-cultural features of mental disorders, the more current trans-, inter- and ethnocultural research were becoming. Thus, in 1967 P. Yap identified "culture-specific syndrome" — a term used to refer to mental illness, specific to a particular region. In 1985, N. Wig introduced the concept of "psychocultural syndromes" [16].

Studies devoted to the search for differences in psychotic symptoms, depending on the place of residence, ethnicity, and age of the respondents are still interesting. The obtained findings may seem unimportant from the point of view of immediate practical importance; however, they represent a great interest for understanding of cultural and other differences because psychosis makes them more obvious.

Researches of the cultural specificity of painful experiences have a long history. Thus, in 1904 while visiting Jawa island, Kraepelin found that in the development of dementia praecox, catatonic symptoms and auditory hallucinations are less common among the natives, but they more often experience confusion in comparison with patients in Europe [40]. Typical clinical picture of psychosis on the Mauritius Island has been described by A. Raman and H. Murphy (1971). It has been found that the level of excitation of the residents of Mauritius was higher and the level of negative symptoms is lower in comparison with schizophrenia patients in England. It was also found that the prognosis of patients with schizophrenia and schizophreniform disorders among population of Mauritius is more favorable comparing with European and American patients [43]. Similar studies were conducted by W. Pfafflin and E. Wulff [53]. They noted the rareness of main symptoms of schizophrenia in Asian countries. All the researchers distinguished specific manifestations of the disorders in different cultural environments. M. Katz, K. Sandhorn and H. Gudeman (1970) paid attention to the fact that Philippino or Polynesian patients mainly had symptoms of arousal, and among Japanese patients' manifistation of autism was dominated [39]. M. Opler (1956) and J. Singer (1956) studied the differences among American citizens of Italian and Irish descent, they found a significantly higher level of imagination and motor activity in Irish patients [49]. Hypochondriacal depression in North Africa is the most severe disorders of consciousness and catatonic syndromes of schizophrenia were described by Wittkower E. and Okashe  A. [45; 52].

The descriptive nature of most of the early publications in the field of psychiatry was based on the observation of random clinical cases. Even with volunteers, experimental studies are impossible because of the ethical issues. However, in those cases when life itself provides the necessary information, and researchers could and have to study data very carefully.

One of the evidence, of socio-cultural influence on the formation of hallucinatory-paranoid symptoms is the effect of hallucinogens in different temporal and cultural contexts. It has been studied that a system of beliefs, motivation, mood,character traits of an individual and his/her physical condition influence on the symptomatology under the affect of hallucinagens [27; 28]. It has been proved that specialities of a culture plays a crucial role in the formation of individual perception of a person and even more, they can program them. The influence of a culture on visions was described caused by hallucinogens. For example, for a good luck in hunting, men of the amahuaca tribe recreated movements and actions of animals in their visions by using the properties of hallucinogens. It was found that they studied features of animal behavior, hallucinations using the content for later conscious recalling. Shamans of different tribes did it in a similar way [34].

Historical time could be considered as a part of a culture. Significant events change the content of psychopathological symptomatology, reflecting what happening in the world: current problems and new discoveries [30], the political situation and religious beliefs [47], refracting through the prism of a mental disorder.

Psychoreactive effect of the historical time is reflected in the following clinical case.

Patient A is thirty-four years old man. The onset of the disorder started in 2001. During the present of the disorder, persistent and systematized hallucinatory-delusional symptoms were formed. Delusions of attitude, influence, poisoning and special power were recognized in a paranoid content. He believed that his sister put feces into his food because she is jealous to his special power. He also felt the influence in different forces. The clinical picture had been stable for 13 years, until in 2014 when he claimed that he defines events in the world. The patient was sure that he changed the political situation in the United States and Kyrgyzstan. He claimed that by using his ability, he evoked a hurricane in 2008 in the United States and thereby destabilized the situation in the country. The patient A. assured that he affected on the presidential election in the United States. Since 2015 the clinical picture of the paranoid schizophrenia became more politicized. He believed that with a help of V.V. Putin conducting two revolutions in Kyrgyzstan (History of illness N 2650-315, 2014; History of illness N 209, 2015).

Psychiatrists know the reflection of political events in the patient's symptomatology very well. Nevertheless it should be mentioned that such reflection has its own system — the more significant the historical event for the future is, the more likely it will be reflected in the delusional content [48].

The content of a delusion reflects not only culture of a specific time and significant socio-political events, but also reflects those features of social life that usually are not covered by the press, as well as the prevailing fears and superstitions, aggressive revival of religion, ancient rituals and paganism. Delusional content reflected the topic of syphilis in the early 1900s, German Nazism during the Second World War [30], the pursecution of spies from hostile states in the 50s of the last century [Ibid], being followed by the Soviet state security Committee [14], restructuring and gangsterism in early '90s, as well as revolution and ethnic tensions in the beginning of the XXI century [13].

The scientist from South Korea, Kwangiel Kim (2001) conducted series of researches studying the impact of culture on psychotic symptoms. In his articles he provides a number of illustrative examples of the political influence on a delusional content among patients with paranoid schizophrenia. Thus his patients claimed that they were being followed by spies from North Korea. The author supposed that in these cases, delusion of persecution reflects the military and political threat from North Korea of that time. In the article it was also written that from 1960 to 1980 patients with delusions related to сommunist espionage in China and Taiwan were often encountered. Recently, the "political delusion" encounters less often and, according to the author, it happens because of the better relationship between these two countries [47].

The following clinical case illustrates the information above.

Patient B. is forty-eight years old. Onset of the disorder was in 1989. Persistent, systematic delusional ideas of attitude, influence and damage were formed. In 2009, new events were wedded, delusional ideas widened because of the delusions of special power and grandeur. He considered himself a criminal lord, who was highly respected by the most important mafia bosses. The patient stated that he constantly visits "gatherings", and solves the criminal problems. He believed that he was persecuted by agents of the National Security Council and by the police, he saw conspiracies, traces and evidence of persecution (History of illness N 345, 2010).

It was the time, when unstable political situation lead to a high level of crime in the Kyrgyz Republic. The two following cases from our practice illustrate political delusions of persecution.

Patient C. is twenty-four years old man. The first clear psychotic episode was observed in 2008. Initially, expansive delusions dominated in the delusional content. The patient thought that he is a very significant person in a business and politics. He said that he was able to communicate with deputies telepathically and created plans for raising economy with Bakiyev's family. In 2009, delusional ideas of persecutory for political reasons were added to the content. He said that he was pursuing by the security services by request of Zhanysh Bakiyev. The patient C. started to notice that people in suits and constantly pursuing tinted car (History of illness N 82, 2009).

It should be mentioned that by 2009, the obvious weakness of the Bakiyev's government, along with blatant corrupt practices with members of his family, gained increasing popularity in public discourse. A paradox, typical for reflection of historical events in the symptoms of paranoid schizophrenia lies in a logical reflection of the most significant moments in a paralogous interpretation.

Patient D. is a thirty years old man. Symptoms of the disorder appeared in 2011. Systematized delusions had been formed for four years. The content includes delusional ideas of grandeur and special significance. He considered himself as a member of security services, claiming that some people watched him, listened to and recorded his conversations. The patient said that he was sent to prevent someone’s attempt to kill the president. He was constantly talking about the national duty, about a plan of an attempt and a coup d'etat. According to the patient because of the political persecution some staff would come for him and they are fake of the government. In the beginning the content of the delusion consisted of expansive ideals, but ideas of persecution was added very quickly and it could be related to political instability and recent revolutions (History of illness N 3044/357, 2015).

We insist that content of the main symptoms of schizophrenia is always related to reality. However, except the cases described above, more cases are encountered in practice, the content of which includes unrealistic, fantastic, fairy-tale and religious characters. Thus, recently there are more and more patients of Kyrgyz ethnicity who have dominating ideas of influence from supernatural characters. According to one of the doctors of the Republican Center of Mental Health: "so many jinns have never been before". Jinns inhibition and influence on thoughts, feelings, submission to the will of the supernatural powers, control his / her actions are often rationalized as a "test", the result of which the person will get some special abilities, such as "kyrgyzchylyk" or "kөzү achyk". It seems that explanation of special popularity of mythical characters in the clinical picture of paranoid schizophrenia lies on the surface. Since the late 90-ies of the last century, at a time of chaos, political and social instability, and extreme uncertainty in the future, there was a renaissance of traditional spiritual practices, including rituals of jinns exorcism, responsible for the evil thoughts, desires and actions. The rituals of exorcism conducted by a healer — a specialist in expulsion of jinns in a mosque, or in a holy place. Compulsory part of the ritual — the use of a whip, by which a jinn (or jinns) is expelled through the patient's body. It should be noted that states of possession are considered not only in the context of paranoid schizophrenia, but also in dissociative disorders. In these cases, a jinn serves as a symbolic representation of a repressed material, or as an isolated, poor or awkward parts of the personality. It is not accident that during the epidemic of conversion disorder in the Caucasus [8], North Kazakhstan [15] and in other post-Soviet countries, exorcism became a mass therapeutic event. In 2009 the center of Islamic medicine was opened by a priest and head assistant of the Chechen Republic in Grozny — Dawood Selmurzaev, and the most popular procedure in the center is driving out of jinns.

The following clinical case shows the reflection of an ethnic difference in the clinical picture and the transformation of delusional symptoms.

Patient E. is 33 years old man. There is an evidence that his grandfather "a well-known moldo in the village". The symptoms of the disorder appeared in 2009. He had sleep disturbances at night, stopped caring about himself, became uncommunicative, angry and conflict person. In the clinical picture of the patient, verbal pseudo-hallucinations of imperative and commenting character were observed. He expressed delusional ideas of reference and influence toward his family, saying that "this is an influence of jinns who possessed him because of a wrong way of living and because his wife is impure." For two years he had been receiving some "specific treatment" from grandfather-moldo, who predicted his "enlightenment", but because the mental state of the patients was detoriating, he was hospitalized twice in the Republic Center of Mental Health in the department for patients with acute states in 2011. After leaving the hospital he started to visit "holy places in the south of the republic", and continued "the treatment" from his grandfather. In 2014, the clinical picture has been changed, he started wearing a specific clothing, grew a beard and behaving provocatively and arrogant. Delusions of grandeur and special powers were added to the delusional content. He said that he is a prophet and an enlightened person now, can treat people, read their minds and able to predict the future. He started to beat his wife in order to cleanse her from jinns. He said that his relatives are also needed too be cleanse from jinns as well (History of illness N 1209/190, 2016).

According to Marek Krzystanek [46], changes of religion in society lead to changes of hallucinations and delusions with religious content.

Delusions reflect the major changes in the religious community. It is important to note that, after visiting holy sites, the patients started burn and give their things, explaining that "due to their unclean houses and their things are also defiled and God told them to do that in order to clean up their surroundings". They expressed delusional ideas, arguing that "it is necessary to share with others, as in Christianity. Parents live in the wrong way, jinn and shaitans are in them".

Due to the unstable position of religion in the country, the emergence of new religious movements, as well as the strengthening of radical Islam, in the content delusions different spiritual movements started to be mixed.

With the increasing implementations of technologies in people's life, the delusional content includes computers [51], Internet [38], and computer games [35]. According to Vaughan Bell (2005), technological innovations affect not only on the content of delusions, but they also influence on its origin and formation. It is possible to consider the formation of delusions associated with the Internet and new technologies, as a type of the cultural delusions according to DSM V, or as a delusion of influence [38]. A similar opinion was expressed by Vladimir Lerner, who argued that the delusion associated with the Internet is not a new category of delusion, it is a modified version of the persecutory delusion [41]. It is clear that the delusions, which are related to the Internet and computer games, are more common among younger patients.

Patient M. is twenty-years old man, whose disorder started in 2011. Events from modern thrillers and stories from the Internet have been reflected into the delusional content. He was telling about corpses and zombies surrounding him and describing their terrible appearance. The patient felt changes in his body intermittently having lots of saliva in the throat and having horrible eyes. He claimed that they were perusing him. He felt the influence — how they change his thoughts (History of illness N 4862-445, 2012).

A young age is characteristic feature in this case and, what is more important the beginning of the disorde was in the period of active movie relised reflected in the delusional content. There is a large number of online games, the content of which often include apocalyptic stories or fantastic characters, such as zombies, vampires, orcs. It certainly has an affect on the development of the delusional content of the young man.

 

Thus, the qustion about the impact of historical time on the psychopathological content in mental disorders has been arisen since the inception of a society, in which people with mental disorders lived. During the formation of psychiatry as a science, three fundamental theories of etiopathogenesis of mental disorders were dominated: endogenous, exogenous and polyethiological. At the present stage of the psychiatric development, it has been proved that formation of psychopathology is influenced by both endogenous and exogenous effects. Psychosis trasformed the information related to current events, informational technologies and revolutional ideas and they were interwined into the content of psychopathology. Based on the original cultural values, ideals, specific personality traits and historical time, diverse variants of transformations in hallucinatory-delusional symptoms became the content of an experience.
